Now imagine taking all these steps in improper shoes. With hundreds of options available like slippers, chappals, sandals, shoes to stiletto heels, we tend to go for style and design of the footwear as dictated by our favorite actor or actress rather than by comfort and proper fit. Our feet are quite complicated structures which not only propel us forward but also bear our weight, act as shock absorbers and provide the foundation on which the rest of our skeleton is balanced. When our feet are not healthy, our entire body is affected when it comes to gait, balance, stability and our motion. With ever increasing activities of men, more and more problems in our musculoskeletal system (bones and muscles) are being recognized by modern science. Issues pertaining to our foot and ankle like sprains, fractures, arthritis, deformities, sports injuries and diabetic neuropathies which have historically been neglected because of lack of proper understanding, are finally getting solved after a lot of research and surgical experimentation. The anatomy and biomechanics of our foot and ankle are so darn complex because of the sheer number of bones, joints, muscles and ligaments, all working closely in a 3-dimensional space, that a specialty of orthopedics is fully dedicated to this field. Understanding which part of this region is giving pain and discomfort to the patient is the most interesting aspect where the doctor has to think like Sherlock Holmes! And it is not a cup of tea for everyone. To be able to reach that position where we can give a diagnosis with certainty and properly treat the patient, we have to first go through 5 years of medical college; then do a 3-year post graduation in “Orthopedic Surgery” followed by 1-2 years of fellowship in “Foot and Ankle”! Needless to say, that only an orthopedic surgeon who has an in-depth knowledge about this region can come up with a viable solution to patient’s problem. Podiatrists, orthotists, prosthetists, physiotherapists and occupational therapists come into picture only after proper assessment by the foot and ankle orthopedic surgeon, not before that! The management of foot deformities like flatfeet and high arched feet requires not just the correction of the bony alignment but also the balancing of dynamic muscles pulling the foot in multiple directions. Similarly, the treatment of diabetic ulcers and Charcot (where bones just break on their own because the nerves are dying!) is not repeated cleaning of wounds and antibiotics. It is the correction of bones and their position which can only be assessed by an orthopedic surgeon specializing in this field. Who else can do key hole surgeries for sports injuries of foot and ankle other than an orthopedic surgeon trained in arthroscopic surgery? The dictum followed by podiatrists (who are totally different from foot and ankle orthopedic surgeons!) that “a foot should look like a foot” is not entirely true. Sports can place tremendous pressure on our feet, ankles and legs. Running and jumping can generate an impact force through the legs that is many times a person’s body weight. The commonly encountered issues are shin splints, stress fractures of various bones, tendinitis (painful muscle ends) and sprains (torn ligaments) which can keep a player out of field for more than a season. Various designs of the footwear are available each specifically meant for different purpose. Running shoes have plenty of cushion to absorb the shock and are designed for forward motion protecting the front of our feet and heel. Minimalist shoes are light, flexible, don’t have much cushion and give the feeling of running barefoot. Walking shoes are more rigid in the front with slightly rounded sole or a “rocker” bottom to help shift the weight more smoothly from the heels to toes. Trail running shoes have a heavy thread, more heel and side-to-side support to keep us safe while running on uneven surfaces. Court shoes (for sports like basketball, tennis and volleyball) and shoes with cleats/spikes/studs (for football and baseball) are quite commonly used by sportspersons which give good grip over the playing surfaces. High top shoes support our ankle during quick changes in direction and when we jump and land. These modifications in the shoes essentially increase speed, agility and reduce fatiguability. Any person who is getting involved in some sort of sporting activity regularly should get his feet and footwear examined by orthopaedic foot and ankle surgeons periodically. The correct size, shape, design, material, time of buying the shoes and for what purpose, everything matters! A single “sports shoe” can’t be just used for any purpose. The feet of young children are very soft; their bones have not yet grown to their full capacity. There is a possibility that the parents may notice something abnormal in their kid’s feet and they should be concerned about the same. Most commonly parents may see that the feet of their kids are flat i.e., absence of arch on the inner side or they are walking weirdly with their toes pointing outward. Babies are born with flatfeet, a condition which corrects itself by the time the kid turns six. However, some of these children may never develop their arch fully. These are the children who should be examined by the orthopaedic foot and ankle surgeons to decide the future course of action in order to avoid possible consequences. Kids may be born with grossly deformed FOOT, most commonly CLUBFOOT that requires good deal of attention from doctors. Many toddlers continuously walk on their toes, a condition known as toe walking. But this habit usually disappears when they reach the age of three. Any persistence of such abnormal walking necessitates medical evaluation. In-toeing, also known as pigeon toes, wherein children walk with their feet turned inward may be caused by various issues up in leg or hip regions. Children may suffer from plantar warts caused by a virus, which causes cauliflower like outgrowths on the sole. These are quite contagious and require medical treatment. As the children grow, there is often a mismatch in the size of nail bed and the overlying nail. This causes the nail to push into the skin on the sides leading to pain and possibly infection, a condition known as ingrown toenails. Children quite frequently suffer from heel pain which NEEDS to be addressed based on its cause. Many foot problems can be avoided or better managed by choosing the right shoes. Proper foot care is never at the forefront of our kid’s mind, as they often just want the newest and coolest pair of shoes that features their favourite characters. Apart from obvious deformity and pain, kids may not keep up with their peers or voluntarily KEEP withdrawing from activities they usually enjoy or just do not want to show their feet.
